Labor and Industries: Accident and Injury Applied massage therapy for accident/ injury treatment, rehabilitation, myofascial pain and referred or prescribed therapy for specific conditions. *Please bring information regarding your L & I Accident, or I will be unable to treat you for injuries until we establish full intake. This includes Claim number, Claim representative and contact info for Claim representative, date and time of the incident, Lawyer contact (if applicable), Drivers License, and Prescription from General Practitioner, Chiropractor or Primary Care Physician.
